Ride day forms

• Ride leaders should use this form to keep track of participating riders. Front side is for club members. Reverse is for guests

• Anyone under 18 participating in a club ride must have a signed release form. If you are expecting minors on your ride, make sure you have these forms available.

• Ride leaders should always carry a copy so that all incident details can be recorded at the site.
